The application must be completed by the managers or trustees of the unit trust.
Time limit for making an application (Regulation 4(2))
The application must be made on or before the last day of the first period of account for which approval is sought.
Please complete all sections and submit with a copy of the current trust deed and prospectus (or similar document made available to investors).
